The on-line Matric Application was created using Adobe Acrobat 7 or newer. You must have Adobe Acrobat Reader version 7 or newer to fill the form out on-line. (Follow the Adobe link below if you need to update.)
There are four (4) pages total. The submit button is at the end. Acrobat will create a file called "matricapptab.xml" and attach that to an email message. Adobe will ask if you want to submit via a PC-based email (Microsoft Outlook, Lotus Notes, etc.) or a web-based email (Yahoo, Hotmail, Gmail, etc.)
This employer participates in E-Verify.
NOTE: If you are using webmail - you will need to have that email program open in another browser window. (Be sure not to have any messages open, but simply have the mail open.) If Adobe does not automatically attach the file to an email, but asks to save the "matricapptab.xml" file, please save it on your PC and attach it to an email and send it to the email address listed at the top of the application.
